Boosters Liability Membership Form 2011-12 The PAL Outdoor Adventure Club is available to
students attending King, Longfellow, and Willard middle schools as well as Rosa Parks Elementary !! Step 1: Download and print the Boosters Membership and Liability Form as well as the 2011-2012 Calendar Step 2: Fill out the Membership form Step 3: Return your filled out membership form to our office: Berkeley Boosters Association 1642 University Ave. Berkeley, CA , 94703 OR your child can drop off the membership form to the Coordinator at your child's Extended Day Program OR email to fuperesa@berkeleyboosters.org
2011-12 School Year Calendar Downloads:
 We will be Sailing with the Pegasus Project at the Berkeley Marina "K" Dock. In order to participate your child MUST have a Pegasus Project liability waiver on file.  We will be dragon boating with Dragon Max at the Berkeley Marina.
In order to participate in this session your child MUST have a signed liability waiver on file.
